Complete the application
Every language version uses the same Google Form. The form is in English, but you may write your answers in your own language. Sign in with your Google account to complete the form. Use the field guide below if you need help with the English labels.

Apply once you have completed all requirements for your course. Select the correct course and confirm that its requirements are complete. Submitting the form does not by itself confirm approval of your application.
Check your name carefully: enter it exactly as you want it printed on your certificate. Provide a complete mailing address, including postal code and country. Review all details before submitting.
How your certificate is ordered
We collect applications in batches of 10 and send each batch to IFBB headquarters in Spain to order the certificates. Once the certificates have been produced and received by IFBB Nordic Academy, we mail them to the addresses provided. There is no fixed delivery date: timing depends on the next batch, production and postal delivery.

- Full Name (as you wish it to appear on your certificate)
Enter your full name exactly as it should appear on your certificate. - Date of Birth
Enter your date of birth in the format shown in the form. - Profession (optional)
Enter your profession. This field is optional. - Nationality
Enter your nationality. - Street Address (house/apartment, street, number)
Enter your complete street address, including any apartment or unit number. - City
Enter the city or town for your mailing address. - State/Province/Region
Enter the state, province or region if needed for your mailing address. This field is optional. - Postal/ZIP Code
Enter your postal or ZIP code. - Country
Enter the country where your certificate should be mailed. - Telephone Number (with country code)
Enter your telephone number with the country code, for example +358. - Which IFBB Nordic Academy course are you applying for a certificate in?
Select the course you completed. Course options use their official English names. - When did you complete this course?
Indicate when you completed the course, following the instructions in the form. - Confirmation of Completed Requirements
Select the same course and confirm that you have completed all of its requirements. - Additional Notes (optional)
Add any information relevant to your application. This field is optional. - Student Declaration (e-signature)
Read the declaration. Type your full name as your electronic signature to confirm that the information you provided is true and correct. - Consent for Data Processing (GDPR)
Read the consent statement in the form and indicate your consent to the processing of your certificate application data.
